Health Screening 40+: what's included and where to get it in Slavuta
Health Screening 40+ includes blood pressure and pulse measurement, weight, height and waist measurement, a questionnaire about your lifestyle and symptoms, a health risk assessment and at least two blood tests: a lipid profile and glycated hemoglobin (HbA1c). It's free for Ukrainian citizens aged 40 and over, who apply for the funds in the Diia app or at an administrative services center (TsNAP). At Viva Clinic in Slavuta, it's done at 3 Danyla Halytskoho St, usually in one visit, and you book through the online form on the website.

Who can get Health Screening 40+
Since 2026, Ukraine has had a state health screening program for people aged 40 and over. You apply for the funds in the Diia app or at an administrative services center (TsNAP) at any time, and the application isn’t tied to your birthday. The screening is free for you: the money to pay for it is credited to your Diia.Card. You can have a state-funded screening once per calendar year.
After 40, high blood pressure, high cholesterol and raised blood sugar become more common. At first these changes usually cause no symptoms, and a person can feel fine for years. The screening helps assess your health and spot risks before you notice anything wrong.
The check is especially useful if you smoke, don’t move much, are overweight, or have relatives with heart disease or diabetes. If you haven’t checked your blood pressure and blood tests for a while, the screening is a good reason to do it.
What Health Screening 40+ includes
The check consists of an exam, a questionnaire and at least two blood tests:
- blood pressure and heart rate measurement
- weight, height and waist measurement
- a questionnaire about your lifestyle, possible symptoms, mood and anxiety
- a health risk assessment
- a lipid profile, a blood test for cholesterol and other fats
- a glycated hemoglobin (HbA1c) blood test
Measuring blood pressure helps catch hypertension, which can go unnoticed for a long time. Weight and height together show whether you’re overweight, and waist size shows fat around the belly, which raises the risk of diabetes and heart disease.
The lipid profile shows the level of cholesterol and other blood fats that affect the risk of atherosclerosis. Glycated hemoglobin reflects your average blood sugar over the past 2–3 months, so it can catch prediabetes or diabetes early. If there are medical reasons, such as high blood pressure or diabetes, the doctor adds kidney function tests, an ECG, or home or 24-hour blood pressure monitoring.
The doctor assesses your risks using all the data together: the same cholesterol level means a different risk for someone who smokes and someone who doesn’t. What the numbers on your results mean and which reference values doctors use is explained in Health Screening 40+ results: how to read your lipid profile and HbA1c.
How to apply in Diia and book
At Viva Clinic, the screening is carried out by doctors of Viva Clinic PE (Ministry of Health license No. 601 of 16.06.2016). Here’s the process:
- Apply for the funds in the Diia app or at a TsNAP. In Diia, open the “State assistance” section (Допомога від держави) and choose the “Health screening” service (Скринінг здоров’я).
- Within 7 days, the funds to pay for the program will be credited to your Diia.Card. A new card or one you already have will work. You need to use the funds within two months, or they go back to the state budget.
- Book through the online form on the Viva Clinic Health Screening 40+ page, on Helsi or by phone.
- Come at your appointment time.
The exam, questionnaire and blood tests are done in one place during one visit. You may need to come back if your blood pressure is high and the doctor gives you a blood pressure monitor for home readings.
How to prepare for the visit
When you book, ask whether you need to come on an empty stomach: under the program rules, the lipid profile can be done without fasting, and glycated hemoglobin hardly depends on food. The rest is simple:
- don’t smoke or drink coffee for half an hour before the visit, as both raise blood pressure for a short time
- arrive a little early so you can sit quietly for a few minutes before the measurement
- wear a top with sleeves that roll up easily: the blood pressure cuff goes on your upper arm, and blood is taken from a vein in your arm
- bring a list of the medicines you take regularly and any previous test results
- don’t stop any prescribed medicines on your own before the screening
Previous results help show how your numbers have changed over the years. If you measure your blood pressure at home, bring those readings too.
What to do with your screening results
During the screening, the doctor will assess your health risks based on your measurements and answers. Even if everything is normal, keep your results: the next time you have tests, the doctor can compare the numbers and see how they’re changing.
A result outside the normal range is not yet a diagnosis. High blood pressure or cholesterol can often be lowered by changes in diet and activity, with treatment prescribed by a doctor if needed.
The doctor who carried out the screening will explain your results and, if needed, give you an electronic referral straight away. Show them to your family doctor or internist as well. They’ll decide whether you need repeat tests, additional checks or a visit to a cardiologist or endocrinologist. At Viva Clinic, these specialists see patients in Slavuta, and their consultations are paid. A free specialist consultation with an electronic referral is available at a facility that has a contract with the National Health Service of Ukraine (NSZU) for outpatient specialist care. If you have a declaration with a Viva Clinic family doctor, meaning you’re registered with them as your primary care doctor, their appointments, basic lab tests and an ECG are free for you.
Questions and answers
Can I get Health Screening 40+ if I'm over 50?
Yes. The program is for everyone aged 40 and over, so being 50 or 60 is no obstacle. You can apply for the funds in the Diia app or at a TsNAP at any time, with no need to wait for your birthday.
Should I wait for the screening if something is already bothering me?
No. The screening helps spot risks when nothing is bothering you. If your blood pressure is rising, or you have pain, weakness or other symptoms, book an appointment with an internist or a specialist without waiting for the screening.

Through the online form on the Health Screening 40+ page of the Viva Clinic website, on Helsi or by phone. Before you fill in the form, apply in the Diia app or at an administrative services center (TsNAP) so the funds for the screening reach your Diia.Card.
