The free state Health Screening 40+ checks blood pressure, weight, health risks, cholesterol and glycated hemoglobin (HbA1c) in people aged 40 and over. At Viva Clinic in Slavuta, you complete it at 3 Danyla Halytskoho St, usually in one visit. First apply in the Diia app or at an administrative services center (TsNAP), then fill in the online booking form on this page.

From 2026, Ukraine has a state health screening program for people aged 40 and over. You apply for the funds in the Diia app or at a TsNAP at any time, and the application isn’t tied to your birthday. The screening is free for you: the funds to pay for it are credited to your Diia.Card. You can have a state-funded screening once per calendar year.
The screening helps assess your health and spot risks when nothing is bothering you. If you already have symptoms, don’t wait for the screening and see an internist or a specialist.
At Viva Clinic, the screening is carried out by Viva Clinic PE doctors (Ministry of Health license No. 601 of 16.06.2016). Here’s the process:
Your blood pressure, heart rate, weight, height and waist circumference will be measured. You’ll answer questions about your lifestyle, any symptoms, your mood and anxiety, and the doctor will assess your health risks.
Then you’ll have at least two blood tests. A lipid profile shows your cholesterol and other fats. The second test measures glycated hemoglobin (HbA1c). It all usually happens in one place and in one visit. If there are medical reasons, such as high blood pressure or diabetes, the doctor adds kidney function tests, an ECG, or home or 24-hour blood pressure monitoring.
It's free for you. This is a state program: once you apply in the Diia app or at a TsNAP, funds to pay for the screening are credited to your Diia.Card within 7 days. You can use a new card or one you already have.
At any time once you're 40 or older, as the application isn't tied to your birthday. You apply in the Diia app or at a TsNAP, and a state-funded screening is available once per calendar year. After that, fill in the online screening booking form on the Viva Clinic website.
Through the online form on this page, on Helsi or by phone. First apply in the Diia app or at a TsNAP so the funds are credited to your Diia.Card. Then choose the way to book that suits you, and you'll be given a visit time at Viva Clinic.
Usually just one. The exam, questionnaire and blood tests are done in a single visit to Viva Clinic. You may need to come back if your blood pressure is high and the doctor gives you a blood pressure monitor for home readings. When you book, ask whether you need to come on an empty stomach: under the program rules, the lipid profile can be done without fasting, and HbA1c hardly depends on food.
